Are Kaguya and Tsukasa the Same Person?
No, they are not. At the beginning of the series, Nasa likened his wife during the accident to the Princess of the Moon, Kaguya. However, it is later revealed in the original manga that Tsukasa is not Kaguya; however, her immortality is somewhat connected to the moon princess.

The Tale of the Bamboo Cutler is a story that tells the life of Kaguya-hime, little did Tsukasa know that she will later become a part of it. Tsukasa was like any normal girl before. Iwasaka, Tsukasa’s father, was a loyal subordinate of the Emperor. When Kaguya returned to the moon she left the Emperor a letter and an immortality elixir. The chances of them reuniting in the future is zero; hence, the Emperor did not want to take the latter and live a life without Kaguya.
The Emperor ordered Iwasaka to burn the elixir at the top of a mountain, but this time he did not obey this task because of her daughter. Her father made her take the elixir and was able to avoid dying. However, Tsukasa’s life never becomes the same way again after taking it.
She was confused and mad. Tsukasa questioned her father’s decision that night for she herself told him that she was ready to die. The elixir that granted her eternal youth only brought despair into her life. She was hunted down by the same people she helped. She felt betrayed and hopeless. To escape the people’s evilness, she had to fight them even if it meant that she would become the monster.

Does Nasa Know That Tsukasa is Immortal?
Tsukasa eventually reveals to her husband that she has lived long before Nasa was even born. But, he was able to know her past even before they got married. Their fated encounter during the night of the accident, Tsukasa already told Nasa who she really is. But, because of the wounds, he got, Nasa forgets about it and only remembers all of it, including his promise to her, 147 chapters later.
In addition, Tsukasa told Nasa more about her past when her closest friend, Tokiko Tsukuyomi, died. Nasa may have been clueless for many months regarding his wife’s identity, but later on, knew about it and still accepted her.
